Registration Begins 11 November and ends 31 December 2008.
REGISTRATION INFORMATION
Please use a separate form for each class. One student per application. This form may be duplicated. To register by mail, please complete and return this form along with your check to the Springfield Art Museum 1111 East Brookside Drive, Springfield, MO 65807 on or after 11 November. Register in person Tuesday - Friday 9:00 - 5:00. Register on Saturday 9:00 - 5:00 or Sunday 1:00 - 5:00 by dropping off a check and the registration form in an envelope marked Museum Educator. Telephone and email registrations cannot be accepted. The Museum reserves the right to cancel any class due to insufficient enrollment. The Springfield Art Museum prohibits discrimination on the basis of race, color, national origin, sex, disability, or age. No class registration refunds will be given after the first week of class.
